Requests via ChartSwap
As of March 2, 2020, all requests for the release of protected health information and billing records are processed through ChartSwap's secure portal.
Written authorization
All patient information is confidential and cannot be released without consent of the patient or their legally authorized representative (unless authorized by law). Requests must be made in writing, dated, and signed. For a minor, a parent or legal guardian must sign.

A current fee schedule is available via ChartSwap. No fee is assessed when information is sent to another health care provider or for a Social Security disability claim (which must include a supporting letter from the SSA). Patients are only charged the fee associated with ChartSwap; all others are invoiced through ChartSwap.

Complete the authorization
Download the authorization form, then fill it in, date it, and sign it (a parent or legal guardian signs for a minor).
Submit through ChartSwap
Create a request on the secure ChartSwap portal and upload your signed form.
Receive your records
ChartSwap notifies you when your records or billing information are ready and delivers them securely.
